Qualifications

• Active LPN license in good standing (state of practice required).

Responsibilities

• Provide direct nursing care to residents in a long-term care setting, including monitoring health status and performing routine nursing tasks.
• Administer medications and treatments as prescribed, following physician orders and safety protocols.
• Monitor for changes in resident conditions, respond to clinical alerts, and communicate with the supervising RN and care team.
• Assist residents with activities of daily living (ADLs) and maintain a safe, supportive environment.
• Document care accurately in the electronic medical record and participate in interdisciplinary rounds and care planning.
• Collaborate with licensed nurses, physicians, therapists, and other staff to deliver comprehensive resident care and ensure regulatory compliance.
