Qualifications

Responsibilities

• Monitor vital signs, perform routine assessments, and report changes to the care team.
• Assist in developing and updating individualized care plans with the interdisciplinary team.
• Communicate effectively with RNs, physicians, residents, and families; respond to call lights promptly.
• Maintain safety, infection control, and standard precautions; assist with activities of daily living as needed.
