Healthforce is seeking a Licensed Practical Nurse for a long-term care setting in Springfield, Vermont. This travel, temporary assignment begins 10/27/2026 and runs for weeks with guaranteed 40 hours per week and weekly pay between $1,824 and $1,897.

Evening and/or Overnight shifts are available. The role requires an active Vermont PN license, experience in long-term care, and strong communication and teamwork to support residents and collaborate with the care team.

Qualifications

• Active Vermont Practical Nursing license is required.
• Experience in long-term care setting preferred.
• Strong communication and teamwork skills required.

Responsibilities

• Administer medications and treatments per physician orders; document in EHR.
• Monitor resident status with vital signs, assessments, and observations; report changes promptly.
• Assist with ADLs, mobility, hygiene, nutrition support, and wound care; ensure resident comfort and safety.
• Perform routine procedures within scope and policy; support infection control and safety standards.
• Collaborate with the interdisciplinary team to develop/revise care plans; handoff to next shift.
• Maintain accurate documentation and ensure regulatory and privacy compliance.

• Administer prescribed medications and treatments per physician orders, ensuring accuracy and safety; document in the electronic health record.


• Monitor resident status with regular vital signs, assessments, and observation; report changes promptly to the supervising RN.


• Assist with activities of daily living, mobility, hygiene, nutrition support, and wound care; maintain resident comfort and safety.


• Perform routine procedures within scope and facility policy; support infection control and safety standards.


• Collaborate with the interdisciplinary team to develop and revise care plans; provide clear handoffs to the next shift.


• Maintain accurate documentation and ensure compliance with regulatory requirements and privacy standards.
