Job summary

Responsibilities

• Monitor vital signs and coordinate with the interdisciplinary team.
• Assist residents with activities of daily living and safety.
• Ensure accurate charting and infection control adherence.

Responsibilities

• Administer medications and monitor patient responses under RN supervision

• Perform routine nursing duties including wound care, toileting assistance, and hygiene support
• Monitor vital signs, report changes, and coordinate with the interdisciplinary team
• Assist residents with activities of daily living and promote safety and comfort
• Ensure accurate charting, medication reconciliation, and adherence to infection control protocols

Requirements

• Active Practical Nurse license (state of licensure appropriate for practice)
• Graduation from an approved practical nursing program
• Basic Life Support (BLS) certification
• Prior long-term care or skilled nursing experience preferred
• Ability to work the 18:00–06:30 night shift in a travel assignment
• Strong communication and documentation skills
