Job summary

Naven Health is seeking an Infusion Nurse I to deliver specialized infusion care in the patient’s home or alternate infusion sites. You will work under close supervision to ensure patient safety and adherence to physician plans of care.

Responsibilities include initiating nursing plans, administering treatments, documenting care in real time, and coordinating with the Clinical team to optimize patient outcomes. Travel 100% may be required.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum 0-2 years of infusion nursing experience.
• Current CPR certification required (in-person class per company policy).
• Must have valid driver’s license and vehicle for business travel.
• Proficient with computers and basic software.

Responsibilities

• Initiates, develops, and implements basic nursing plan of care treatments, evaluating patient progress towards goals.
• Performs treatments, administers medications, and educates patients and families/caregivers.
• Modifies plan of treatment in response to changing patient status based on physician orders to achieve goals.
• Assesses patient needs and physical status at each visit and communicates with the clinical team to coordinate care.
• Completes clinical documentation in real-time during visits and supports billing processes.
• Under supervision, coordinates discharge planning and provides patient instructions and follow-up as applicable.
• Oversees paraprofessionals in the home setting as applicable and participates in multidisciplinary team discussions.
