Job summary

Gentiva Hospice in Milwaukee is seeking a Hospice RN – Regional Support to provide direct patient care and family support while serving as a floating clinician across branches based on census needs. You will travel to Gentiva hospice locations to fill urgent needs and collaborate with physicians and care teams.

The role emphasizes delivering compassionate, high-quality nursing care, coordinating plans of care, and participating in on-call coverage with eventual regional responsibilities.

Qualifications

• Active RN license in the state of employment.
• Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology or other high acuity setting).
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Deliver high-quality, compassionate nursing care for hospice patients.
• Manage and implement individualized plans of care.
• Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.).
• Assess and address patients’ physical, emotional, spiritual, and environmental needs.
• Accurately document all care provided and ensure compliance with hospice protocols.
• Educate families and caregivers on care instructions, medications, and safety.
