Job summary

Gentiva is seeking a Hospice RN – Regional Support to deliver high-quality nursing care and support to families across multiple Western TN locations. You will rotate to fill urgent needs, travel within the region, and participate in after-hours care as part of an on-call rotation.

The role emphasizes comprehensive patient assessment, care plan implementation, and collaboration with physicians and care teams to ensure compliant, compassionate hospice services.

Qualifications

• Current, unencumbered RN license in the state of employment.
• Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care or oncology).
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Provide direct, compassionate nursing care to hospice patients.
• Develop and implement individualized plans of care.
• Lead/interdisciplinary team members (LPNs, CNAs, HHAs, etc.).
• Assess and address patients' physical, emotional, spiritual, and environmental needs.
