Job summary

Gentiva Hospice is seeking a Hospice RN – Regional Support to deliver direct patient care and support families while floating among branches to cover census needs. The role combines clinical duties with coordination across teams in multiple communities.

The position requires travel, adherence to hospice protocols, and participation in on-call coverage. An unencumbered RN license, CPR certification, and reliable transportation are essential for success.

Qualifications

• Current, unencumbered RN license in the state of employment.
• Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology or other high acuity setting).
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Deliver high-quality, compassionate nursing care for hospice patients.
• Manage and implement individualized plans of care.
• Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.).
• Assess and address patients’ physical, emotional, spiritual, and environmental needs.
• Educate families and caregivers on care instructions, medications, and safety.
• Participate in the on-call rotation and provide after-hours care as needed.
