Job summary

Vitality Hospice is seeking an Admissions Registered Nurse (RN) to join our clinician-owned hospice team. This on-the-road role requires travel within service areas and offers a competitive salary and comprehensive benefits.

You will perform comprehensive admissions and patient/family assessments, document care plans, administer medications as needed, and collaborate with the interdisciplinary team to tailor plans of care. Excellent communication and organizational skills are essential.

Qualifications

• Active RN license required.
• 1+ year of skilled nursing experience preferred.
• Hospice or admissions experience preferred (not required).
• Willingness to travel within assigned service areas.
• Strong communication, organizational, and interpersonal skills.
• Valid driver''s license, reliable transportation, and auto liability insurance.

Responsibilities

• Complete comprehensive hospice admissions and patient/family assessments.
• Ensure accurate documentation and appropriate care planning.
• Provide direct patient care as needed, including medication administration and treatments.
• Collaborate with physicians and the interdisciplinary team to develop individualized plans of care.
• Serve as a liaison between patients, families, and healthcare providers.
• Educate patients and caregivers on disease progression, symptom management, and hospice services.
