Qualifications

• Graduate from an accredited School of Nursing. Associate’s Degree in Nursing, required. Bachelor’s Degree in Nursing, preferred.
• Minimum 1-2 years of professional nursing experience in the related specialty.
• Current license to practice as a Registered Professional Nurse in New York State and/or Connecticut, required.
• BLS required, plus specialized certifications as needed.

Responsibilities

• Assesses the patient’s physiologic health status and plans care.
• Assesses patient’s psychosocial health status and coping needs.
• Identifies patient outcomes and develops individualized goals.
• Formulates the plan of care and establishes priorities.
• Implements the plan of care and ensures safety and timely care.
• Evaluates care and revises the plan based on outcomes.
• Collaborates with the care team and physicians during treatments.
• Performs Grade I-IV decubitus care and related skin care tasks.
• Administers medications correctly and safely.
• Participates in patient and family education and discharge planning.
• Performs related duties as required.
