Qualifications

• Current license to practice as a Registered Professional Nurse in New York State.

Responsibilities

• Assesses the patient’s physiologic health status and documents plan of care.
• Assesses patient’s psychosocial health status and supports patient education.
• Develops patient outcome statements and care plans.
• Implements and documents the plan of care with safety and efficiency.
• Administers medications correctly and safely and educates patients and families.
• Performs discharge planning and coordinates referrals for follow-up care.
