Job summary

Qualifications

• Graduate from an accredited School of Nursing.
• Current license to practice as a Registered Nurse in New York State.
• BSN preferred.
• 2 years of Med/Surg or Telemetry experience highly preferred.

Responsibilities

• Assess the patient's physiologic health status.
• Assess the patient's psychosocial health status.
• Identifies patient outcomes and develops outcome statements.
• Formulates the plan of care and sets priorities to achieve outcomes.
• Implements the plan of care and coordinates care delivery.
• Evaluates care and documents patient responses and outcomes.
• Collaborates with physicians and health team members in planning and carrying out treatment.
• Provides patient and family education and discharge planning.
