Job summary

Northwell Health is seeking a Registered Nurse to provide age-specific, evidence-based nursing care across a designated patient group. You will assess physiologic and psychosocial health, plan and implement care, and coordinate with physicians and the health team to meet patient outcomes.

You will lead care delivery, document interventions, support discharge planning, and educate patients and families. NY license and BLS are required; Bachelor’s in Nursing is preferred.

Qualifications

• Graduate from an accredited School of Nursing.
• Associate’s Degree in Nursing required.
• Bachelor’s Degree in Nursing preferred.
• Current license to practice as an RN in New York State and/or Connecticut; BLS required.

Responsibilities

• Assess the patient’s physiologic health status and plan of care.
• Assess psychosocial health status and support needs.
• Develop patient outcome criteria and individualized goals.
• Formulate and coordinate the plan of care with the care team.
• Implement treatment plans and monitor patient safety and comfort.
• Provide discharge planning and patient/family education.
• Evaluate care effectiveness and document outcomes and interventions.
• Collaborate with physicians and care team members to adjust care.
