Job summary

Rocky Mountain Care is seeking a Personal Care CNA to provide compassionate in-home and facility-based support in Woods Cross, Utah. You’ll assist with daily living activities, monitor client conditions, and follow personalized care plans to promote safety, comfort, and independence.

Qualifications

• Current CNA certification in State Name.

• Valid driver’s license, reliable transportation, and willingness to travel to client locations.

Responsibilities

• Assist clients with activities of daily living (ADLs) — including bathing, grooming, dressing, toileting, and feeding.
• Support mobility, positioning, and safe transfers.
• Provide light housekeeping, laundry, and meal preparation as needed.
• Offer companionship, conversation, and emotional support.
• Monitor and report changes in client condition to the care team.
• Maintain accurate, timely documentation of care provided.
• Follow each client’s personalized care plan with attention and empathy.
