Qualifications

• Minimum of 2 years of inpatient medical‑surgical experience or acute rehabilitation within the last three years.
• Possess a current unrestricted license to practice as a registered nurse.
• Basic Life Support certification required.

Responsibilities

• Implement and oversee Case Management activities for assigned caseload.
• Assess patients' clinical and psychosocial status and coordinate medical treatments.
• Ensure compliance with established standards and guidelines.
