Job summary

Centre For Neuro Skills in Plano, TX is seeking a Clinical Case Manager to oversee the development, implementation and management of all clinical aspects of rehabilitation services. You will bridge clinical and administrative matters across clinics and residential sites, coordinating care and ensuring quality outcomes.

The role requires a Bachelor's in a related field, 3–5 years in rehabilitation, and CCM certification preferred.

Qualifications

• Passion for care and ability to communicate effectively with patients and families.
• Leadership skills to recruit, train, and evaluate staff and make tough decisions when needed.

Responsibilities

• Develops and manages clinical case management plans.
• Plans objectives and actions to meet the patient’s needs; communicates and facilitates goals with staff.
• Coordinates with external stakeholders and families to ensure coordination of services.
• Implements case management activities to achieve plan goals.
• Evaluates plan effectiveness at intervals and updates as needed.
