Job summary

Centre for Neuro Skills is seeking a Clinical Case Manager to lead the development, implementation, and management of clinical case management for patients with traumatic brain injury. The role bridges clinical and administrative work across clinic and residential sites, with a Monday–Friday schedule and some on-call duties.

The ideal candidate has 3–5 years of rehab experience, a related bachelor's degree, and CCM certification preferred.

Qualifications

• Bachelor’s degree in a related field required; master’s or licensure preferred but experience can offset this level.
• Minimum of 3–5 years’ experience in rehabilitation or related field.
• CCM certification preferred; demonstration of capacity to absorb clinical knowledge is valued.

Responsibilities

• Collect information about the patient’s situation and functioning to identify needs and develop a case management plan.
• Plan objectives, goals and actions to meet patient needs; communicate goals with staff.
• Communicate with external stakeholders and families to coordinate services.
• Implement case management activities to achieve plan goals.
• Evaluate plan effectiveness and modify components as needed.
