Brain Injury Services in Northern Virginia seeks a full-time Brain Injury Case Manager to support adults and children with brain injuries, coordinating services, documenting assessments, and navigating Medicaid referrals. Travel to clients' homes or communities is required; a valid driver's license and reliable transportation are essential.

Qualifications

• Excellent oral and written communication skills and the ability to de-escalate misunderstandings.
• Strong time management, organizational, and documentation skills.
• Knowledge of ADA and IDEA, long-term case management, and Medicaid/MCO processes.
• Strong analytical and problem-solving abilities.
• Ability to prioritize work, manage caseloads, and travel within service areas.

Responsibilities

• Adhere to professional standards, federal and local requirements, and MCO regulations to serve and protect the agency.
• Obtain information and make referrals for appropriate services, including TCM-related referrals.
• Complete intake evaluations, obtain medical records, and determine service eligibility.
• Process Medicaid TCM paperwork, authorizations, assessments, and billing.
• Develop Individualized Service Plans and review ISP every 90 days with client signatures.
