Job summary

Advocate Health is seeking a Lead role at the Trauma Recovery Center (Condell Medical Center) to guide inpatient care for trauma patients and coordinate multidisciplinary collaboration. The position focuses on scheduling, on-call planning, and ensuring data quality while supporting a team of clinicians in a fast-paced hospital setting.

The role requires strong communication, three years of social services experience, and the ability to adapt to varying shifts including evenings and weekends.

Qualifications

• High School Diploma or GED required.
• Minimum of three years of experience in social services.
• Strong verbal and written communication, ability to work in a team and independently.
• Proficiency in Microsoft Office Suite, strong organizational skills, and professionalism.

Responsibilities

• Assist leadership with daily operations and team performance to meet goals and maintain productivity and quality documentation.
• Provide immediate emotional support and resources to hospitalized trauma patients; assist with discharge planning and psychoeducation groups.
• Coordinate schedules with hospital and trauma clinic teams; monitor productivity and on-call schedules.
• Lead outreach and maintain connections with community partners to support continuity of care.
• Schedule patient appointments and ensure follow-up communications after discharge.
