Advocate Health seeks a Lead role to support inpatient trauma care teams, coordinating schedules and resources while ensuring data quality and productive operations. The Lead supports leadership by providing onsite guidance and support to the team responsible for inpatient care for individuals who have experienced recent trauma. This role assists with on-call schedules, fosters collaboration with internal and external stakeholders, coordinates hospital and trauma clinic schedules, ensures data quality, monitors productivity, and fulfills other key responsibilities under the direction of the leadership team.
Assiststheleadershipwith daily operations and team performance by guidingthe teamto meet individual and team goals,high standardsof productivity, quality documentation, interdisciplinary collaboration, and fostering a supportive work environment.
Provides immediate emotional support and resources to hospitalized patients followingatrauma, including connecting them with the clinical teamwhenappropriate,assistingwith discharge planning, leading support and psychoeducation groups, and safety planning.Also, complete assessments and facilitatewarm-handoffsto providers for ongoing care.
Responds to trauma alerts,safety calls, pre-escalation needs andcollaborates with street outreachteams andfacilitates mediation tomitigateretaliationrisks.
Guides the team, offers patient support, works alongside other groups, and actively engages in intensive community outreach. This also involves making follow-up telephone calls and sending letters to discharged patients to connect them with outpatient resources.
Schedules patient appointments with the TRC, other internal Advocate Health resources,or community partners before dischargeand provides brief follow-up calls afterdischargewhen necessary, to ensure ongoing carewhich requires strong collaborationwith other providers.
Conducts intakes and needs assessments, linking patients to appropriate resourcesand outpatient services.It may also require notifying other hospital providers of unmet needs to be addressed while in the hospital.
Maintainsaccuraterecords of patient interactions and action plans.
Works with the hospital treatment team and community partners to ensure comprehensivetreatment, discharge planning,and continuity of care.

Associate’s Degree
Three ormore years of experience in social services.
Epic or EMR experience
Previoussupervisory experience.
Experience working with trauma-affected populations.
Bilingual
Full-time position (40 hoursper week), with flexibility to cover all shifts as needed.
