Qualifications

Masters or Bachelors in a healthcare related field or related experience.
Associates in a healthcare field with 6 months related experience.
Associates in non-healthcare with 1 year related experience.
Certified Medical Assistant with 1 year related experience.
Certified Peer Support Specialist with 1 year related experience.
High School diploma or GED with 2 years related experience.

Responsibilities

Perform assertive community outreach with hospitals, shelters, faith communities and other partners.
Build and maintain professional relationships with community groups, service organizations and law enforcement.
Provide outreach and clinical case management aligned with treatment plans.
Assist member eligibility for financial entitlements and support enrollment.
Accompany members to appointments and interviews with agencies.
Provide follow up on referrals to assess outcomes and additional services.
Screen individuals for TRC service eligibility and make referrals.
Coordinate intake with CODAC staff until enrollment is completed.
Address barriers to treatment engagement and maintain records of outreach.
