Advocate Aurora Health is seeking a dedicated trauma recovery support specialist for on-site, full-time work in Christ Hospital's Trauma Recovery Center. The role involves engaging with patients who recently experienced intentional trauma and coordinating with the trauma medical team, TRC, and community resources to reduce re-injury risk.

Responsibilities include trauma-informed emotional support, safety planning, discharge coordination, and broad outreach to connect patients to outpatient

Qualifications

• Strong verbal and written communication skills.
• Ability to work collaboratively within a team and independently.
• Proficiency in Microsoft Office Suite.

Responsabilités

• Provide immediate trauma-informed emotional support, advocacy, and resources to hospitalized patients following trauma, including assisting with discharge planning and connecting to outpatient providers.
• Respond to trauma alerts and safety needs; collaborate with outreach teams to mitigate risks.
• Engage in intensive community outreach, outreach calls, and connect patients to resources and wraparound services.
• Schedule patient appointments to ensure continuity of care with TRC and community partners.
• Conduct intakes and needs assessments; notify providers of unmet needs.
• Maintain accurate records of patient interactions and action plans.
• Collaborate with hospital teams and community partners for comprehensive care.
