Job summary

Advocate Health is seeking a Trauma Recovery Specialist to provide immediate trauma-informed emotional support and advocacy for patients recently experiencing intentional trauma. The role involves connecting patients to clinical teams, coordinating discharge planning, and facilitating access to outpatient resources within the Advocate Trauma Recovery Services network.

As a frontline team member, you will engage in outreach, safety planning, and psychoeducation while collaborating with hospital

Qualifications

• High School Diploma or GED required.
• Minimum of one year experience in social services.
• Strong verbal and written communication skills.
• Ability to work collaboratively within a team and independently.
• Proficiency in Microsoft Office Suite.
• Strong organizational skills.
• Ability to handle difficult conversations and maintain professionalism.
• Willingness to adapt to evolving job responsibilities.

Responsibilities

• Provide trauma-informed emotional support, advocacy, and resources to hospitalized patients.
• Coordinate discharge planning and connect patients to outpatient care.
• Conduct trauma outreach and connect patients to community resources.
• Schedule patient appointments to ensure ongoing care and continuity of services.
• Maintain accurate records of patient interactions and action plans.
• Collaborate with hospital teams and community partners to ensure comprehensive care.
