Responsibilities

• Perform initial and ongoing patient assessments and document health status.
• Develop and implement nursing plans to achieve outcomes as part of a multidisciplinary team.
• Evaluate response to care and revise the plan as needed to ensure quality and safety.
• Lead or participate in process improvement to support clinical quality and safety.
