Responsibilities

• Review referrals to determine readiness for transitional living after interview and referral.
• Carry a caseload of approximately 15 residents.
• Develop an Individualized Service Plan with residents and review it quarterly.
• Communicate regularly with the Family Support Team and share progress/concerns.
• Connect residents to community services as needed.
