Lad Lake, Inc. in Elkhorn, Wisconsin is seeking a Community Therapist (PT) to join a trauma-informed, multidisciplinary team serving youth and families across several counties.

The role includes in-home, virtual, or in-office therapy, maintaining detailed documentation, and coordinating treatment plans. The position emphasizes flexibility, travel across counties, and a commitment to equitable, client-centered care.

Qualifications

• Master’s degree in social work, counseling, or related field.
• Active licensure required (LPC, LCSW, LMFT, or Training License).
• Deep understanding of trauma-informed care and youth development.
• Willingness to travel across assigned counties.
• Valid driver’s license and acceptable driving record.

Responsibilities

• Conduct initial clinical assessments and develop individualized treatment plans.
• Provide therapy to youth and families in-home, virtually, or in-office based on clinical need.
• Advocate for youth and families within community systems.
• Manage and revise crisis response plans as needed.
• Maintain accurate and timely documentation of all clinical services.
• Collaborate closely with interdisciplinary treatment teams.
• Complete treatment updates and discharge summaries in a timely and comprehensive manner.
• Refer clients to appropriate community resources as needed.
• Perform other related duties as assigned.
