Job summary

Lad Lake Inc in Port Washington, Wisconsin, is seeking a Community Therapist (PT) to deliver trauma-informed therapy to youth and families across multiple counties. The role includes in-home, virtual, or in-office sessions with flexible hours and travel required to serve communities in need.

Applicants should hold a Master’s degree in social work, counseling, or a related field and an active license (LPC, LCSW, LMFT or Training License).

Qualifications

• Master’s degree in social work, counseling or related field required.
• Active licensure (LPC, LCSW, LMFT or Training License) required.
• Trauma-informed care and youth development experience preferred.
• Ability to travel across assigned counties and work flexible hours.

Responsibilities

• Conduct initial clinical assessments and develop individualized treatment plans.
• Provide therapy to youth and families in-home, virtually, or in-office.
• Act as advocate for youth and families within community systems.
• Maintain accurate documentation of clinical services and collaborate with treatment teams.
• Complete treatment updates and discharge summaries; refer to community resources as needed.
