Job summary

Lad Lake, Inc. is seeking a Community Therapist (PT) to deliver trauma-informed care to youth and families across Wisconsin. The role involves in-home, virtual, and office-based therapy within a multidisciplinary team, with a focus on individualized treatment planning and advocacy.

The position offers a flexible schedule, travel across assigned counties, and opportunities for professional growth within a long-standing Wisconsin nonprofit dedicated to youth well-being.

Qualifications

• Master’s degree in social work, Counseling, or related field.
• Active licensure required (LPC, LCSW, LMFT, or Training License).
• Trauma-informed care and youth development understood.

Responsibilities

• Conduct initial clinical assessments and develop individualized treatment plans.
• Provide therapy to youth and families in-home, virtually, or in-office based on clinical need.
• Advocate for youth and families within community systems.
• Manage and revise crisis response plans as needed.
• Maintain accurate and timely documentation of all clinical services.
• Collaborate with interdisciplinary treatment teams.
• Complete treatment updates and discharge summaries in a timely and comprehensive manner.
• Refer clients to appropriate community resources.
• Perform other related duties as assigned.
