Job summary

Lad Lake, Inc. in West Bend, Wisconsin is seeking a Part-Time Community Therapist to deliver trauma-informed therapy to youth and families across several counties.

In this home-/office-/virtual-based role, you will conduct assessments, develop treatment plans, collaborate with multidisciplinary teams, and maintain documentation, with occasional evenings and travel. Minimum requirements include a Master’s degree in social work or related field and active licensure (LPC, LCSW, LMFT, or Training

Qualifications

• Master’s degree in social work, Counseling, or related field.
• Active licensure required (LPC, LCSW, LMFT, or Training License).
• Trauma-informed care and youth development knowledge.
• Experience working with youth and families.
• Willingness to travel across assigned counties and some evenings.

Responsibilities

• Conduct initial clinical assessments and develop individualized treatment plans.
• Provide therapy to youth and families in-home, virtually, or in-office based on clinical need.
• Maintain accurate and timely documentation of all clinical services.
• Collaborate closely with interdisciplinary treatment teams.
• Complete treatment updates and discharge summaries in a timely and comprehensive manner.
• Stay current on and refer clients to appropriate community resources.

Education
