Qualifications

• Two years of experience with case management or a related discipline is required.
• Must have a valid driver's license.
• Ability to communicate effectively both verbally and in writing.

Responsibilities

• Receives/responds to incoming calls from referral sources/potential clients.
• Completes all viable referrals by setting up and carrying out pre-screening assessments.
• Oversees field staff assisting with the pre-screening process.
• Consults with third-party representatives regarding client benefit coverage.
• Contacts referral sources to advise them of case acceptance.
• Maintains relationships with standard referral sources.
• Monitors and tracks referral sources' satisfaction levels.
