NYC Administration for Children’s Services seeks a Community Coordinator to oversee case management for residents, coordinating mental health referrals and court-ordered adjustments. Qualifications

  • A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities.
  • High school graduation or equivalent and six years of experience in community work or community centered activities.
  • Education and/or experience equivalent to the above, with at least one year of relevant experience.

Responsibilities

  • Meet with residents at assigned hall(s) to discuss needs and progress.
  • Complete Case Management Intake and PREA for new residents.
  • Document progress notes and summaries of discussions.
  • Prepare court-ordered adjustment reports.
  • Communicate with parents weekly and issue parental notifications as needed.
  • Send emails and correspondence with attorneys per residents’ requests.
  • Refer residents to mental health services as needed.
  • Liaise with Mental Health, DOE, Group Services, External Providers and Medical staff.
  • Advocate for youth to ensure access to services while in detention.
