Job summary

HEALTH CARE FOR THE HOMELESS INC is seeking a licensed social worker to provide direct behavioral health therapy and intensive case management for adults experiencing homelessness. You will develop individualized treatment plans, coordinate with landlords and care teams, and support housing stability through evidence-based practices.

Ideal candidates hold an MSW, are LMSW/LCSW-C licensed, and have access to a vehicle for community visits in Maryland.

Qualifications

• Master’s Degree from an approved School of Social Work.
• Current LMSW or LCSW-C license.
• Personal vehicle and valid Maryland driver’s license.

Responsibilities

• Provide mental health therapy and intensive case management to adults experiencing homelessness.
• Conduct home and community visits and accompany clients as needed.
• Collaborate with an interdisciplinary care team and external agencies.
• Develop integrated treatment plans using SMART goals with clients.
• Ensure authorization and billing processes are completed per guidelines.
• Document in the electronic health record clearly and timely.
