Job summary

Crossroads Health seeks a Supported Employment Case Manager to assist adults in obtaining and maintaining employment while managing mental health symptoms. You will work in home, community, or office settings and build relationships with local employers to place clients in suitable jobs.

Responsibilities include collaborating with a team, coordinating with agencies such as OOD, and applying trauma-informed practices to integrated behavioral health and recovery services.

Qualifications

• Three years related experience with High School Diploma or equivalent, or Associate’s degree.
• Minimum (1) year experience within mental health field (preferred)
• QMHS within 10 days of hire (employer provided)
• Valid Driver’s License
• Must have an acceptable driving record (4 points or less) and reliable transportation with insurance
• BLS/CPR/First Aid (as applicable, company provided)

Responsibilities

• Coordinate Employment Services and support within home, community, office or workplace for adults needing assistance in obtaining/maintaining employment while managing mental health symptoms.
• Develop relationships with employers throughout the community.
• Engage as a member of a collaborative team and possibly work with TY, forensic or dual populations.
• Coordinate with outside agencies to provide services such as OOD.
• Apply a Trauma-Informed approach to integrated recovery and mental health services.
