Job summary

LifePath Systems seeks a dedicated professional to perform outpatient behavioral health services with a trauma-informed, person-centered approach. The role involves managing a caseload and traveling across Collin County to individuals in homes, community settings, nursing facilities, and hospitals.

You will coordinate with LifePath teams and community partners to ensure timely access to employment supports and reduce barriers to care.

Qualifications

• Bachelor’s degree in psychology, social work, or related field from a four-year college or university.
• At least two years of experience in working with the community, preferred (i.e., social services, recovery, customer service, sales).
• Bilingual (Spanish/English) preferred.
• Must have reliable transportation and a valid driver license.

Responsibilities

• Perform outpatient behavioral health services to individuals across LifePath system of care.
• Maintain a caseload and travel to homes, community settings, nursing facilities and hospitals within Collin County.
• Facilitate access to employment supports and reduce barriers to obtain employment.
• Coordinate services with BH Division and community organizations to improve outcomes.
• Provide care coordination with trauma-informed and person-centered approach.
