Job summary

Crossroads Community, Inc. is hiring a Client Support Specialist to join our Psychiatric Rehabilitation team. You will assess clients, develop IRPs, coordinate services, document progress, and help them gain independence and resiliency in daily life.

Responsibilities include maintaining medical records, collaborating with families and other providers, and transporting clients as needed. We seek compassionate, reliable individuals dedicated to empowering clients through person-centered support.

Qualifications

• High School Diploma or equivalent.
• AA or BS in Behavioral Health, Psychology, Social Work, or related field and 2 years of positively referenced work history in human services.

Responsibilities

• Assess clients' strengths, needs, resources, and desires, and help develop their Individual Rehabilitation Plan (IRP) as frequently as required or needed.
• Coordinate services provided by Crossroads for assigned clients.
• Assist clients in meeting their goals and assess and document their progress each month.
• Assist clients to develop resources needed to promote recovery and resiliency and life skills necessary to maintain independent living and success in life.
• Maintain clients’ medical records in accordance with regulatory and accreditation requirements.
• Utilize computerized databases or other procedures to submit accurate, timely and professional documentation of services in accordance with agency policies, regulations and accreditation standards.
• Routinely collaborate with family/caregivers as allowed and other providers of services to clients.
• Provide transportation to clients when needed.
