Responsibilities

  • Provide individual counseling to assigned clients.
  • Review and evaluate intakes and assessments as assigned.
  • Formulate individualized rehabilitation plans.
  • Serve as a member of the interdisciplinary team.
  • Provide group education to clients as assigned.
  • Attend staff meetings and maintain client records.
