Qualifications

• Four years of progressively responsible experience in social services
• Associate degree in a related field with three years of experience in social services
• Bachelor's degree in a related field with two years of experience in social services
• Any equivalent combination of education, training and experience demonstrating ability to perform the duties

Responsibilities

• Interviews and elicits pertinent information from applicants
• Conducts comprehensive assessments to identify strengths and needs
• Interprets eligibility rules for programs
• Develops individualized case plans with goals and success strategies
• Provides referrals and information about other service providers and available assistance
• Supports clients in meeting short and long-term goals
• Maintains funding allocations and prepares records/reports
• Coordinates services with other agencies and may conduct home visits
• Attends required meetings and trainings
