Job summary

Coconino County is seeking a dedicated Case Manager to support individuals and families in navigating social services. You will interview, assess, plan, and coordinate resources to help clients meet short- and long-term goals.

The role requires strong communication, cultural sensitivity, and the ability to work with diverse populations. A fingerprint clearance card and Arizona driver's license are required.

Qualifications

• Four years of progressively responsible experience in social services.
• Associate degree in a related field with three years of experience in social services.
• Bachelor's degree in a related field with two years of experience in social services.
• Any equivalent combination of education, training and experience.
• Fingerprint clearance card required.
• Bilingual (Spanish and/or Navajo preferred).
• Must obtain an Arizona driver's license by date of hire.

Responsibilities

• Interview applicants and elicit pertinent information for assistance.
• Assess individuals and families to identify strengths and needs.
• Interpret eligibility rules for a range of programs.
• Develop individualized case plans with goals and success strategies.
• Provide referrals and information about other service providers and resources.
• Coordinate services with other agencies and prepare records and reports.
• May conduct home visits and attend required meetings.
