The Retreat Inc. in Suffolk County seeks an Outreach and Support Specialist to expand awareness of abuse in later life, build partnerships, and provide trauma-informed case management to adults 50+.

The role involves travel to Riverhead, East Hampton, and other Suffolk locations, delivering education and connecting survivors to safety and resources. Responsibilities include crisis intervention, intake assessments, service planning, advocacy, referrals, data management, and grant reporting.

Formación

• Bachelor's degree in a related field preferred.
• At least 2–5 years in client services, outreach, or advocacy.
• Trauma-informed approach and cultural sensitivity required.

Responsabilidades

• Provide trauma-informed crisis intervention and case management to survivors age 50+.
• Complete assessments and intakes and develop individualized service plans based on needs.
• Provide advocacy, safety planning, referrals, and follow-up.
• Assist clients in navigating housing, healthcare, legal, financial, and public benefit supports.
• Maintain a database of resources to support older adults experiencing abuse.
• Coordinate internal and external referrals.
• Maintain accurate client records.
