Job summary

Walker’s Point Youth & Family Center (WPYFC) seeks a compassionate Outreach Case Manager to serve youth and young adults (11–25) at risk of or experiencing commercial sexual exploitation and trafficking. The role blends street outreach with Center-based services to connect participants to housing, healthcare, education, employment, and legal resources.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Criminal Justice, or related field preferred.
• Equivalent combination of education and significant related experience considered in lieu of a degree.
• Minimum of 2 years of experience with populations including trafficking survivors, youth homelessness, or mental health/substance use challenges.

Responsibilities

• Participate in scheduled street outreach efforts alongside Inner Beauty Center staff.
• Engage youth and young adults in locations where they congregate using trauma-informed practices.
• Build trusting relationships and provide crisis intervention and emotional support during outreach contacts.
• Distribute basic needs supplies and assist in Drop-In Center operation.
• Maintain active caseload up to 18 clients and develop individualized service plans.
• Assist participants in housing, mental health, medical, employment, and legal services.
• Advocate with landlords, employers, schools, and healthcare providers.
• Conduct home and community visits and accompany clients to appointments.
• Maintain confidential client records and timely documentation.
• Participate in coordinated care meetings and interdisciplinary teams.
