Qualifications

• Master’s Degree in Social Work (MSW) or Mental Health Counseling (MHC) from an accredited program.
• Two years post-graduate experience preferred in medical/mental health setting.
• Licensed or eligible LMSW/LCSW/LMHC in NYS.

Responsibilities

• Conduct psychosocial assessments and contribute to individualized treatment plans.
• Provide individual, family, and group therapy and crisis intervention.
• Document services timely and thoroughly for all communications.
• Collaborate with interdisciplinary team to coordinate patient care.
• Serve as liaison with families, outpatient providers and supports.
