Qualifications

• Master of Social Work degree from an accredited institution and licensure in NY (LCSW or LMSW).
• Three (3) years post graduate clinical experience in a mental health setting.

Responsibilities

• Completes assessments of new patients and documents history, symptoms, and background.
• Assesses mental status and risk, communicates with other providers as needed.
• Develops individualized written treatment plans with measurable goals and discharge criteria.
• Reviews and updates treatment plans every 3 months with patient involvement.
• Provides individual, group, and/or family psychotherapy as outlined in plans.
• Handles scheduling, billing, and documentation including insurance authorizations.
