Job summary

Cascadia Health's Plaza Health Center in SE Portland seeks an Information and Referral Specialist to manage referrals, enroll clients in Respite services, and complete intakes within a trauma-informed environment.

You will coordinate appointments across mental health, substance use, gambling, and primary care, using the EHR system, and provide exceptional customer service while adhering to HIPAA and safety protocols.

Qualifications

• High school diploma or equivalent required; certifications preferred.
• Experience handling referrals, intakes, and trauma-informed care.
• Ability to work within HIPAA guidelines and emergency protocols.

Responsibilities

• Schedule and route appointments across all health centers using the EHR system.
• Complete pre-registration and intake processes with accuracy.
• Provide trauma-informed customer service and support crisis referrals.
• Maintain compliant documentation in EHR and ancillary records.
