Job summary

USA Health on the Gulf Coast seeks a qualified social services professional to support patients and families. The role involves needs assessments, care planning, and direct counseling to address goals and challenges.

You will coordinate referrals with social workers, maintain EMR records, and participate in team meetings to ensure coordinated care. Minimum HS diploma with 1 year of relevant experience; a BA in a related field is preferred.

Qualifications

• High School Diploma or equivalent with 1 year of experience working with at-risk youth or individuals recovering from trauma; counseling experience required.
• Bachelor's Degree in social work, psychology, counseling, or related field preferred.
• Comparable education/experience may substitute for required qualifications.

Responsibilities

• Conduct comprehensive needs assessments with patients and families to identify strengths, challenges and goals.
• Develop collaborative case plans outlining interventions and support services.
• Provide direct services to patients, including individual counseling or participation in groups.
• Collaborate with social workers to facilitate referrals to community resources (financial aid, housing, job training).
• Work with the social worker to address challenges during case management.
• Participate in team meetings and case staffing to ensure coordinated care.
• Maintain accurate EMR documentation and ensure compliance with standards.
• Complete all mandatory department, educational and hospital requirements.
