Job summary

Illumination Health + Home in California is seeking a Housing Case Manager to provide comprehensive case management for individuals experiencing homelessness or at risk of homelessness. You will advocate, coordinate resources, and support clients toward housing stability and permanent housing.

The role requires maintaining a caseload of 30-35 clients, developing individualized housing plans, and documenting in the EHR/HMIS. Salary offered is $22–$25 per hour, with day hours 8:00 AM to 4:30 PM.

Qualifications

• High school diploma or equivalent required.
• Professional verbal and written communication skills essential.
• Access to a vehicle and valid CA driver’s license; proficient use of Microsoft Office/Teams.

Responsibilities

• Maintain a caseload of 30–35 clients and develop individualized housing plans.
• Engage with clients weekly and document in EHR/HMIS within 24–48 hours.
• Coordinate housing resources, referrals, and community supports with partners.
• Attend weekly Interdisciplinary Care Team meetings and track progress.
