Job summary

Illumination Health + Home is seeking a Housing Case Manager to provide comprehensive case management for clients experiencing homelessness or at risk. You will assess needs, coordinate resources, and help clients achieve housing stability.

Responsibilities include maintaining a 30–35 client caseload, developing housing plans, weekly engagement, and documenting in the EHR. Benefits include strong medical coverage and professional development opportunities.

Qualifications

• CA driver’s license required and eligible for company vehicle insurance.
• High school diploma or equivalent.
• Strong verbal and written communication skills.
• Must have access to a vehicle.
• Proficiency with Microsoft Office/Teams.

Responsibilities

• Maintain a caseload of 30–35 clients and develop individualized housing plans.
• Conduct assessments and coordinate housing resources for clients experiencing homelessness or at risk.
• Engage with clients weekly and document interactions in the EHR within 24–48 hours.
• Assist with unit applications, transportation to viewings, and landlord referrals.
• Attend Interdisciplinary Care Team meetings to track goals and outcomes.
• Link clients to healthcare, food, hygiene items and psychosocial supports.
