Qualifications

• 2+ years of experience in case management working with geriatric populations.
• Proficiency in electronic record-keeping.
• Experience working with Holocaust survivors or other trauma-impacted populations.

Responsibilities

• Provide trauma-informed, patient-centered case management services.
• Assist in organizing socialization events for survivors.
• Document client interactions in a timely manner according to guidelines.
• Manage a large caseload while ensuring high-quality service delivery.
• Address and mitigate client crises with professionalism.
