Job summary

Qualifications

• Bachelor’s degree required; MSW preferred.
• One year related experience and/or training.
• Proficiency in Microsoft Office Suite and related technology.
• Excellent communication and listening skills.
• Excellent customer service skills with a focus on treating clients with respect and dignity.
• Able to multi-task and work independently with great attention to detail.

Responsibilities

• Conduct intakes, provide accurate information and referrals.
• Screen and assist clients with applications and re-certifications for entitlements and benefits.
• Assess clients in home, hospital or other settings as appropriate.
• Advocate for clients and arrange services; develop and follow up on care plans.
• Provide supportive counseling to clients as needed.
• Maintain ongoing communication with client, family and collaterals.
• Maintain complete and accurate case notes and prepare required documentation.
• Maintain and report statistics according to Selfhelp standards.
• Participate in supervision and staff meetings and share emergent issues.
• Facilitate and assist at social, recreational and group activities.
• Supervise social work students (if applicable).
• Adhere to organizational trainings on Sexual Harassment Prevention, IT Security Awareness, Compliance, OSHA, HIPAA.
