The Joseph P. Addabbo Family Health Center, Inc. is seeking a Licensed Clinical Social Worker to provide comprehensive clinical and psychosocial services to individuals and families affected by HIV/AIDS in New York.

The role includes therapy, assessments, crisis intervention, care coordination, and linkage to community resources. The Social Worker will work with an interdisciplinary team to deliver trauma-informed, culturally responsive, and strengths-based care, with special attention to

Qualifications

• MSW from CSWE-accredited program and NY LCSW licensure required.
• Active NYS licensure or eligibility for social work licensure in accordance with regulations.
• Experience with HIV/AIDS, behavioral health, or medically complex populations preferred.
• Experience providing individual and/or group therapy, psychosocial assessment, crisis intervention, care coordination, and community-based services preferred.
• Strong communication, documentation, advocacy, and interdisciplinary collaboration skills.

Responsabilités

• Provide individual and group therapy, psychosocial support, and clinical interventions to patients and families.
• Conduct psychosocial and behavioral health assessments to identify needs and goals.
• Develop and implement individualized treatment and service plans with ongoing follow-up.
• Provide crisis intervention and emergency psychosocial support as needed.
• Coordinate care and link patients to medical, behavioral health, and community resources.
• Address aging-related psychosocial needs for older adults living with HIV/AIDS.
• Collaborate with medical and behavioral health teams to coordinate integrated, patient-centered care.
• Participate in community outreach, health education, and stigma-reduction activities.
• Maintain timely, accurate clinical documentation per agency standards.
• Participate in interdisciplinary meetings and professional development.
• Assist with special projects as assigned.
