Job summary

APLA Health is seeking a PSS Social Worker to coordinate care for patients with psychosocial stressors affecting HIV care. The role focuses on engagement, case planning, and connection to housing, benefits, and support services, both in clinics and in the community.

You will document activities in EHRs, collaborate with medical and behavioral health teams, and ensure HIPAA compliance while delivering trauma-informed care to diverse populations in the Los Angeles area.

Qualifications

• Master’s Degree in Social Work from an accredited program.
• At least one year of care coordination or case management experience in clinical or community settings.
• Experience working with people living with HIV/AIDS preferred.

Responsibilities

• Navigate and link patients to APLA Health resources (Benefits, Housing, HIV+ support).
• Provide referrals, advocacy, and service information as needed.
• Use the Transtheoretical model to support engagement, treatment, and retention.
